One or two floaters for years — usually nothing to worry about. They are shadows cast by clumps in the gel inside the eye. If they appear suddenly, in a swarm or with flashes — get seen today.
This isn’t a diagnosis: the doctor will tell you for sure after examining the back of your eye.
Urgency, honestly
The retina doesn’t hurt. So the warning sign isn’t pain — it’s what you see. The retina is like wallpaper: while only the edge has come away, it can be stuck back. Three signs that mean calling the same day rather than booking for next week.
From above, below or the side, a dark veil covers part of your vision and doesn’t go away when you blink. This is often how a retinal detachment begins.
Yesterday all was clear, today there’s “soot” or a “cobweb” drifting and something sparkling at the side. This happens when the retina starts to tear.
Cover your good eye. If the other one has got noticeably worse in a day or two, or lines in the centre bend — it’s not tiredness.

Treatment path
Treatment is chosen not by the symptom, but by what the doctor sees at the back of the eye. So the first step is the same for everyone — a dilated exam, sometimes an OCT scan too. Then comes one of three paths, each with its own page of details.
First step for everyone
Drops widen the pupil, and through a lens the doctor sees the retina right to its edge — where tears usually begin. OCT images its layers the way a scanner takes a slice: it shows swelling and a membrane in the centre that can’t be seen with the lamp.
Path · laser
When: a tear or thinning at the edge of the retina, diabetic retinopathy, swelling in the centre.
The laser places micro-spots on the retina, and around the weak spot it heals more firmly, like the edge of wallpaper under a staple gun. No incision, sitting at the lamp.
Path · injections
When: swelling in the centre of the retina, wet macular degeneration, diabetic swelling — the things that make straight lines bend.
The medicine goes exactly where it works, not through the bloodstream of the whole body. The eye is numbed with drops, and the needle is thinner than for a blood test.
Path · surgery
When: retinal detachment, blood in the vitreous gel, a tear in the centre, a membrane.
From inside the eye, the surgeon removes the clouded gel that pulls on the retina and puts the retina back in place. In Astana, with no trip to another city.
Clinic price list
The price of laser photocoagulation depends on how much of the retina needs strengthening. A large area is split into stages, and you pay for each one — like renovating a flat room by room. The doctor will tell you how many stages you need after the exam, before treatment starts.

The retina deteriorates silently: there’s no pain, yet vision slips away from the edges or the centre. If you recognise yourself below, a yearly back-of-the-eye exam is like servicing your car: cheaper than a repair after a breakdown.
With diabetes, the retina is checked once a year, even if you see well. Changes in the blood vessels are treated with laser photocoagulation or injections before they reach the centre. A laser stage for diabetic retinopathy is 50,000 ₸ per eye, per stage.
With severe short-sightedness, the retina is thinner at the edges. Weak spots are found at the exam and strengthened in advance with preventive laser photocoagulation, before a tear. If you plan to get rid of glasses, this is done before laser vision correction.

No surprises
Retinal laser photocoagulation is done sitting up, with no incision and no general anaesthesia. You sit at the same kind of lamp as at a normal visit, while the doctor aims the beam at the right spots through a lens.

Questions
One or two floaters that have drifted for years without changing are usually not dangerous: they are shadows from clumps in the vitreous gel. Get them looked at during a routine back-of-the-eye exam. If floaters appear suddenly, in a swarm or together with flashes, come in the same day: this happens with a retinal tear. If long-standing floaters get in the way of reading and driving, they can be cleared with a laser — vitreolysis costs 100,000 ₸ per eye.

No. A detached retina doesn’t go back into place by itself, and drops or tablets won’t stick it back. A tear is sealed with laser photocoagulation while there’s no detachment yet. If detachment has started, vitreoretinal surgery is needed — at RB CLINIC it’s performed by Dr. Zhanysbayev, 1,000,000 ₸ per eye.

The injection price covers the procedure itself. The drug is chosen for the diagnosis: Eylea, Lucentis and Ozurdex work differently and cost differently. The doctor will tell you which one you need and how many injections after the OCT.
Once a year, even if you see well: diabetes changes the retinal blood vessels without pain or floaters. If there are already changes, the doctor will tell you how often to come. A stage of laser photocoagulation for diabetic retinopathy is 50,000 ₸ per eye, per stage.
Better not. For the exam and laser, the pupil is widened with drops, and for a few hours everything is blurry and light is dazzling. Come by taxi or with someone who can drive, and bring sunglasses.
