Bags, fat pads or puffiness
Surgery removes what has pushed forward: fat and excess skin. It won’t treat puffiness from salt, poor sleep or kidney problems. The fat under the lower lid isn’t “extra”: it’s a cushion that holds the eye in its socket. With age, the membrane that holds it back weakens and the fat bulges forward. These are the “fat pads” — bags that stay in the morning and after rest.
The membrane is taut and keeps the fat in place. The under-eye area is smooth, the same morning and evening.
The membrane weakens and the fat pushes forward — that’s a fat pad. A shadow forms below it. Like a pillow bulging through a worn cover: no cream or concealer will put it back.
Lower blepharoplasty removes or repositions the bulging fat and, if needed, trims excess skin.

Surgery
The surgeon removes or repositions the fat and, if needed, trims excess skin. The incision is either just under the lashes or on the inside of the lid, chosen for your case.
The incision runs 1–2 mm below the lash line and hides in a natural crease. Both fat pads and excess skin can be addressed.
The incision is on the inner side of the lid, with no visible stitch outside. Suits cases with bags but little excess skin.
The surgeon will tell you at the consultation which approach suits you.
The operation is two millimetres from the eye, so not only your blood but the eye itself is checked first. No aspirin-containing drugs for a week before; no make-up or contact lenses on the day.

About the risks, honestly. Lower blepharoplasty has known risks: temporary swelling of the conjunctiva, asymmetry, pulling down or turning out of the lower lid if it was lax before surgery. Discuss this with the surgeon at the consultation: he will assess how firm your lower lid is.
