Dacryocystitis is inflammation of the tear sac when tears can’t drain into the nose. Tears pool in the sac like water in a clogged sink, and bacteria start to grow. So the main sign isn’t the tears themselves but pus from the inner corner of the eye.

For parents
In babies, the tear path is most often closed by a single thin membrane where it opens into the nose. It isn’t cut — it’s opened. Before birth the membrane covers the duct, and after birth it should break open by itself. If it stays, tears can’t drain: the eye is wet, there are crusts in the morning, yet the white of the eye is clear.
The doctor presses the inner corner of the eye, sees what comes out and tells dacryocystitis apart from conjunctivitis. Don’t start any drops or ointments without seeing the doctor.
The doctor will show you at the appointment: with a clean finger, gently stroke down from the corner of the eye towards the side of the nose. The pressure from inside often breaks the membrane — and nothing else is needed.
A thin, smooth probe passes along the natural path and opens the membrane. No incision or stitches, and your baby goes home with mum. By the clinic’s price list, probing is 10,000 ₸ per eye.

Step by step, simplest first
Tear duct surgery is the last step, not the first. First, drainage is restored by flushing; if that’s not enough, a probe is passed; and only if the duct is completely blocked is a new path made for the tears. Like the pipe under a sink: first you clear it, and you replace it only when there’s nothing left to clear.
Flushing
A thin, blunt cannula is passed through the tear opening and the path is flushed with a solution, finishing with medication placed right at the site of inflammation. It’s both treatment and a test: if you feel the solution in your nose or throat, the path is open; if it comes back out, the doctor sees where the blockage is.
Probing
If flushing is difficult, the narrowed spot is opened with a thin probe along the natural path, and tear drainage is restored. If the tear opening itself is narrow, it’s widened. No incision or stitches, home the same day.
Surgery
If the duct is completely blocked, there’s nothing to open — the surgeon connects the tear sac directly to the nasal cavity, bypassing the old duct. The operation is called dacryocystorhinostomy. It can be done through the nose, with no skin incision, or from the outside — the surgeon will tell you which you need after the examination.
Not everyone gets as far as surgery: first come flushing and probing, both through the tear’s natural path, with no incision.

Honestly: after flushing and probing the duct can narrow again, especially with chronic inflammation. Then surgery is discussed — the new path doesn’t depend on the old duct.

Often, yes. In babies, the membrane at the duct’s exit is opened with tear sac massage, and if that doesn’t help, with probing. In adults with a narrowed duct, flushing with medication and probing help. Surgery is needed when the duct is completely blocked and flushing doesn’t get through.
Numbing drops are put in before the procedure. The cannula and probe are thin and smooth, and they follow the tear’s natural path, with no incision. You feel pressure in the corner of the eye and the solution in your nose or throat — which means the path is open.
Show your baby to an ophthalmologist: they’ll tell newborn dacryocystitis apart from conjunctivitis (with dacryocystitis the white of the eye is clear) and show you how to massage the tear sac. Don’t start drops without a doctor, and don’t warm the corner of the eye. If massage doesn’t help, the doctor will suggest probing.

It can, if the duct narrows again: this happens with chronic inflammation. Then flushing is repeated or surgery is discussed, which creates a new path for tears from the sac into the nose, bypassing the old duct. That’s why it’s important to finish treating the inflammation and not stop the medication once you feel better: that way drainage lasts longer.
Pus in the tear sac can spread into the tissues of the face: this is tear sac phlegmon — red, hot swelling, pain, fever. A fistula can form. Chronic inflammation keeps the eye constantly wet. With swelling and fever, see an ophthalmologist the same day.
During flushing: you’ll feel the solution, put in through the tear opening, in your nose or throat — meaning the path is clear and tears can drain. If the solution comes back through the same or the neighbouring opening, the doctor sees where the blockage is and decides what to do next.
