Entropion

Entropion: why the lashes rub your eye, and why it matters

With entropion, the lashes rub the cornea every time you blink. Left untreated, this can lead to corneal scarring and ulcers.

Entropion means the eyelid margin turns inward, towards the eye. It most often affects the lower lid in older people: with age the lid’s supporting ligaments stretch, and the muscle rolls the edge inward. Scarring entropion after burns, injuries or inflammation also occurs — then the upper lid is affected more often.

A lash rubbing the cornea leaves scratches on it. Over time this can cause scarring, clouding and even a corneal ulcer, so entropion should not be left to wait.

This is not trichiasis. Sometimes the lid sits correctly but individual lashes grow towards the eye. That is trichiasis, and it is treated differently. The exam shows which one you have.

Ectropion

Ectropion: why the eye waters and turns red

With ectropion, the lower lid pulls away from the eye. The eye closes less well, dries out and waters.

Ectropion usually develops with age, as the lower lid becomes lax. It can also follow scarring, facial nerve palsy or an unsuccessful lower blepharoplasty.

Drops and gels ease the symptoms while you wait for surgery, but they do not put the lid back in place: only surgery corrects ectropion for good.

The surgeon returns the lid margin to its proper position. Usually the lid is tightened and supported, and if there is scarring, the tension is released. Like a curtain that has sagged or twisted: you do not cut it, you rehang it on the rail.

Age-related entropion and ectropion

The lid is usually tightened horizontally: pulled taut and anchored at the outer corner of the eye; sometimes the muscles that pull the lid down are also reattached.

Scarring entropion and ectropion

The lid is released from the scar that pulls it the wrong way. Sometimes a tissue graft is needed — then it is eyelid reconstruction.

Can it be managed without surgery?

Temporarily — yes, permanently — no. Tape that pulls the lid into position, lubricating drops and gels relieve irritation for a while. These are ways to manage until surgery, not a cure: they do not put the lid back in place.
