Two different treatments

How squint is treated: without and with surgery

A squint brings two different problems, and they’re treated differently. The first is position: the eye drifts to one side, and it shows in every photo. The second is vision: the brain stops using the turning eye, and it gets weaker. Glasses and patching treat the vision; eye muscle surgery treats the position. Like a dentist and an orthodontist: both work on teeth, but the job is different.

Occlusion: the healthy eye rests — the turning eye learns to work

A patch or sticker goes over the healthy eye for a few hours a day. The brain has to look through the turning eye, and its vision improves. Patching doesn’t straighten the eye, but without it the eye would stay weak even after surgery.

People put off squint treatment for years because of three phrases they heard from friends. Let’s go through each one — with no promises that can’t be checked.

“It’s too late”

For adults, surgery lines up the eyes at 25 and at 50: the muscles work just as they did in childhood. One honest caveat — 3D vision doesn’t always come back in adults. But you’ll stop hiding your gaze in photos and in conversation.

Squint surgery: how the surgical treatment goes

Squint surgery doesn’t change the eye — it changes the tension of the muscles that turn it. Like a horse’s reins: if one side pulls harder, the head turns aside. The surgeon loosens or tightens the right rein — and the eye looks straight.

  1. Exam and measuring the angle. They check each eye’s sharpness of vision, whether glasses are needed and how the eyes move in every direction, and measure the squint angle. The retina and the back of the eye are examined.
  2. A plan to the millimetre. The surgeon decides which muscles to move and by how much before surgery, not on the table. Sometimes both eyes are done at once: the angle is split evenly.
  3. Surgery. Through a small cut in the conjunctiva the surgeon finds the muscle and moves its attachment point. The eyeball isn’t opened, and the instruments never go inside the eye.
  4. Afterwards. For the first few days the eye is red and a little sore — that’s the cut healing, not a complication. Drops on a schedule, follow-up checks. Children often carry on with glasses or patching so the weak eye can catch up with the strong one.

Types of squint surgery

RecessionWeakens the muscle that pulls harder: its attachment is moved back. A common choice for convergent squint, when the inner muscle overpulls.
ResectionStrengthens a weak muscle: it’s shortened and reattached closer to the cornea. Often used for divergent squint.
Repositioning and transpositionThe muscle’s attachment point is moved to change the direction it pulls. Used for vertical squint and complex cases.
Combined and repeat surgerySeveral muscles in one operation, or a second operation if the angle has come back over time. The repeat operation is a separate line in the clinic’s price list.

What is a squint (strabismus)

A squint, or strabismus, is when the eyes don’t look at the same point. One eye looks at an object, the other looks away. It’s not just about appearance: the brain gets two different pictures and has to cope with them somehow.

Types of squint

  • Convergent (esotropia) — the eye turns towards the nose. The most common type in children.
  • Divergent (exotropia) — the eye turns towards the temple, often more noticeable in the evening.
  • Vertical (hypertropia and hypotropia) — the eye turns up or down.
  • Constant and intermittent — the eye turns all the time, or only when the person is tired.

How a person with a squint sees

Normally both eyes look at one object, and the brain merges two pictures into one 3D image. With a squint the pictures differ. An adult sees double. A child’s brain protects itself differently — it switches off the turning eye, and that eye weakens: this is amblyopia, “lazy eye”. Without 3D vision it’s harder to pour water into a glass, catch a ball or park a car.

When to show your child to a doctor

The eye drifts in photos or when your child is tired; they squint or close one eye in the sun; tilt their head while watching cartoons; bump into corners. In babies’ first months the eyes sometimes “wander” — that’s normal, and a wide nasal bridge can create a false squint. Only an exam can tell a real squint from a false one.

Which squint is harder to treat

Paralytic squint — after an injury or stroke: success depends on whether the nerve recovers. A congenital squint that wasn’t treated in childhood — the eyes can be straightened, but 3D vision is harder to bring back. A large-angle vertical squint more often needs a second operation. That’s why a child’s squint shouldn’t be left to “grow out of”.
