Surgery day
You arrive in the morning and are usually home by evening. Vitrectomy is done under local anaesthesia: the eye is numbed, you’re awake and can tell the surgeon if anything bothers you. It takes from 30 minutes to several hours — depending on what needs to be done inside the eye.

The doctor dilates the pupil and examines the retina. An OCT scan shows its layers: without it the surgeon can’t say exactly what needs repairing.
Drops, anaesthetic next to the eye — and you’re in the operating room. Everything is in one building: no need to travel anywhere.
The surgeon works under a high-magnification microscope: three punctures, clearing, retinal repair, filling. There’s no pain; at most you may feel some pressure.
After a few hours of observation you usually go home. In complex cases you may stay for 1–2 days. The next day — a check-up.
The surgeon decides on the method at the exam. The main one is vitrectomy: removing the vitreous gel and repairing the retina from inside. Alongside it — endolaser photocoagulation (a laser from inside the eye), and gas or silicone oil to hold the retina in place. For some detachments a buckle is placed on the outside of the eye.

What families worry about most
For the first days the eye sees blurrily — that’s how it should be. Vision returns gradually, over a few weeks to a few months. The retina is like fresh plaster: it needs time to set, and while it dries, it’s left alone.
The first days
If there’s gas in the eye, the doctor will ask you to hold your head in a certain position for a few days — for example, looking down. The bubble floats up and presses the retina where it’s healing.
Weeks
Don’t rub the eye, lift heavy things or do strenuous sport. How long for, the surgeon will tell you at the check-up.
While there’s gas in the eye
At altitude the gas bubble expands. Plan any trip to another city after your doctor gives the go-ahead: it’s simpler to have surgery in Astana and stay in Astana for that time.

Vitreoretinal surgery treats the retina and the vitreous — the back of the eye, where neither drops nor an external laser can reach. The vitreous is the clear gel that fills the eye. The retina is a thin layer on the back wall that “takes the picture” for the brain. These are the most common reasons people come.
Retinal detachment
The retina comes away from the wall of the eye like wallpaper from a damp wall. Without surgery, sight in that eye can be lost.
Vitreous haemorrhage
Everything is in a red fog, or the eye barely sees. It happens with diabetes, injury or a burst vessel. The blood is removed and its source is sealed with a laser from inside.
Macular hole
A spot in the middle of whatever you look at: letters disappear, faces are blurred. There’s a tiny gap in the centre of the retina.
Epiretinal membrane
The door frame looks wavy, as if through crumpled film. A thin membrane has grown over the centre of the retina and is pulling on it.
Diabetes
Over the years diabetes changes the blood vessels of the retina. When injections and laser are no longer enough, the scar strands and blood are removed surgically.

Surgery isn’t always needed
